Counsellor: East of Montreal

Counselling-Network

Summary: The counsellor is responsible to help develop and provide the support services related to the Counselling-Network project to the East of Montreal* community. The counsellor will work with caregivers and individuals living with Alzheimer’s disease and other forms of dementia with the goal of bringing support, information, resources and accompaniment. The counsellor will be dedicated to the territory on a full-time basis and will be the principal representative of the Alzheimer Society of Montreal on the territory for the counselling and referral services. The counsellor will develop and provide structured support services and programs; in addition, he/she will forge links with relevant community resources. Reports To: Coordinator, Counselling-Network.

*East of Montreal includes: Rivière de prairie, Pointe aux trembles/Montréal-est, Mercier-est/Anjou, Saint- Léonard, Saint-Michel, Olivier Guimond, Rosemont, et Hochelaga-Maisonneuve.

Duties / Responsibilities:

  • Direct intervention with caregivers
  • Assess the client and his/her needs and develop an intervention plan
  • Accompany and support caregivers
  • Orient and refer clients to appropriate resources
  • Provide information and assist client in adopting appropriate strategies
  • Animate caregiver support groups
  • Manage client confidential information, record keeping and statistics

Development

  • Plan, organize and lead structured programs on territory (Alzheimer Café)
  • Network with relevant organizations to build community resource information and contacts
  • Develop and maintain relationships with contacts in the territory to optimize referrals, collaboration and other projects
  • Deliver presentations about Alzheimer’s disease and the Alzheimer Society of Montreal’s mission and services
  • Learn about new developments by reading professional literature, attending seminars, participating on committees and establishing and maintaining contact with other resources
  • Work in collaboration with the staff, volunteers and community partners to achieve the mission and vision of the Alzheimer Society of Montreal

Position Requirements:

Education and Experience

  • Bachelor’s degree in social work or a related field
  • Work experience with seniors and caregivers: minimum of 2 years
  • Solid understanding and sensitivity to the unique psychosocial issues/dynamics associated with target population
  • Significant knowledge of Alzheimer’s disease and other forms of dementia and caregivers
  • Knowledge of community healthcare agencies and resources
  • Awareness of East of Montreal community culture and needs

Skills:

  • Bilingual, French and English, other languages is an asset
  • Counselling and active listening skills
  • Ability to work autonomously, effectively manage priorities and cooperatively in a team environment
  • Good judgment and diplomacy
  • Good sense of initiative
  • Strong organizational skills
  • Excellent oral and written communication skills
  • Computer literate and comfortable with learning new software programs
  • Ability to present information clearly and effectively
  • Respect for the traditions, values and norms of clients originating from different cultures and ethnicities
  • Enthusiastic, empathetic, and respectful professional who is a positive thinker

Terms: Full time, 35 hours/week

Position requires that the counsellor is able to commute to service points and meeting locations within the territory (East of Montreal).
